Is Hormone Therapy Right for Me? How I Decide

By Barbara W. Turpin, DNP, APRN, FNP-BC

"Is hormone therapy right for me?"

I hear this almost every week, and I'm always glad when someone asks it. It tells me they're not looking for a quick fix. They want to get it right.

Here's my straight answer. For the right person, hormone therapy can be life-changing. For someone else, a different approach works better. An evaluation is the only way to know for sure, but here's how to tell whether it's worth a conversation.

Signs it may be worth exploring

For women, it's usually some mix of hot flashes, night sweats, 3 a.m. wake-ups, mood swings, brain fog, a libido that disappeared, vaginal dryness, or painful sex during perimenopause or menopause. Women who go through menopause early, before 45, may also benefit for their bones and heart.

For men, it's low energy, low drive, erectile changes, losing muscle no matter how hard you work at it, more belly fat, low mood, or trouble focusing. Especially when several show up together.

Who's a good fit?

For women, it's usually someone under 60 or within 10 years of menopause, with symptoms that are getting in the way and no major risk factors.

For men, the key is confirmed low testosterone. That means symptoms plus low levels on more than one morning blood test. Symptoms alone aren't enough. Fatigue and low libido can have many other causes.

When it may not be the right fit

I don't usually recommend hormone therapy for people with certain cancers, a history of blood clots or stroke, or other conditions that raise the risk. Men who want children soon may need a different approach, since testosterone lowers sperm production. I spent nearly nine years in the ICU, so I take this part seriously. I cover it in detail in Is Hormone Therapy Safe?

If it's not hormones, it's still worth knowing

Many symptoms that feel hormonal turn out to be something else. Thyroid problems. Low iron. Blood sugar. Sleep apnea. A medication side effect. Depression or plain old stress. Sometimes the fix is a different medication, a lifestyle change, or treating something else entirely. Either way, you leave with answers.

How we decide together

Your first visit is a real conversation about your symptoms, your history, your medications, and what you want to feel like again. Then every hormone patient gets a baseline lab panel before we prescribe anything. No exceptions. You can read what's in it in Do I Need Lab Work Before Starting Hormone Therapy?

Once the results are back, we sit down and go through your options: the benefits, the risks, and the alternatives. For women, that's usually estradiol as a patch or cream, with progesterone if you still have your uterus. For men, it's usually testosterone as a weekly injection or a daily cream. Some women ask about testosterone too. The evidence supports it only in limited situations, so it's a conversation we'd have after your evaluation, not a starting point.

The decision is yours, made with full information.

A little about me

I'm a board-certified family nurse practitioner and the founder of Trinity Health Plus, with 17 years in healthcare, first in critical care and then in family practice. When you come here, you see me. Every time. No one hands you off, and no one rushes you.

I see patients in person in New Port Richey, FL, and by telehealth in Florida, California, New York, Arizona, and Alaska.

If any of this sounds familiar, let's find out what's going on.

This article is for general education and isn't medical advice. Whether hormone therapy is right for you can only be decided after a full evaluation.
