Trinity Health Plus provides adult primary care, preventive care, chronic disease management, sick visits, medication management, and ongoing care coordination. Insurance may be used for eligible covered medical services. Self-pay options are available for selected noncovered services

SICK & ACUTE CARE VISITS
Same-day or next-available visits when scheduling permits
Respiratory symptoms, urinary symptoms, minor skin concerns, and other common acute issues
Evaluation, treatment, medication management, and follow-up as appropriate
PRIMARY CARE FOLLOW-UP
Follow-up for chronic or acute medical concerns
Medication management and treatment adjustments
Review of labs, imaging, and specialist recommendations
Ongoing preventive care, chronic disease management, and care coordination
TELEHEALTH VISITS
Virtual visits for appropriate primary care concerns
Follow-up for chronic conditions and medication management
Review of laboratory results and treatment plans
Select acute concerns when an in-person examination is not required
